The one where a sad little blob rolled around on screen with a rain cloud hovering over its head? That commercial first debuted back in 2001, and since then Zoloft, or Sertraline hydrochloride, has become one of the top psychiatric drugs used by American adults, according to one recent study. “Zoloft is one of the first-line antidepressant medications prescribed for both depression and anxiety, meaning it’s one of the most likely to work,” says Alison Hermann, M. D., a clinical psychiatrist at Weill Cornell Medicine and New York-Presbyterian Hospital. Zoloft is part of a class of drugs called SSRIs, or selective-serotonin reuptake inhibitors. “Their main effect has to do with changing the signaling of one of the main neurotransmitters in the brain, serotonin, which modulates mood,” explains James Murrough, M. D., the director of the Mood and Anxiety Disorders Program at the Icahn School of Medicine at Mount Sinai. buy viagra derby Initial: 50 mg q Day PO given continuously throughout menstrual cycle or given during luteal phase only May increase by 50 mg at the onset of each new menstrual cycle; no more than 150 mg q Day when administered continuously or 100 mg q Day when administered during luteal phase only 25 mg PO q Day initially; may increase by 25 mg every 2-3 days; not to exceed 200 mg q Day Alzheimer dementia related depression: Start at 12.5 mg/day and titrate every 1-2 weeks to response; not to exceed 150-200 mg Renal impairment: Dose adjustment not necessary Mild hepatic impairment (Child-Pugh 5-6): Decrease recommended starting dose and therapeutic dose by 50% Moderate-to-severe hepatic impairment (Child-Pugh 7-15): Not recommended; sertraline is extensively metabolized, and the effects in patients with moderate and severe hepatic impairment have not been studied Clinical worsening and suicide ideation may occur despite medication Use caution in patients with seizure disorders May worsen mania symptoms or precipitate mania in patients with bipolar disorder Increases risk of hyponatremia and impairment of cognitive/motor functions in the elderly Increases risk of bleeding in patients taking anticoagulants/antiplatelets concomitantly Risk of mydriasis; may trigger angle closure attack in patients with angle closure glaucoma with anatomically narrow angles without a patent iridectomy Pregnancy: Conflicting evidence regarding use of SSRIs during pregnancy and increased risk of persistent pulmonary hypertension of the newborn (see Pregnancy) In neonates exposed to SNRIs/SSRIs late in third trimester: Risk of complications such as feeding difficulties, irritability, and respiratory problems Avoid abrupt withdrawal Bone fractures reported with antidepressant therapy; consider the possibility if patient presents with bone pain, bruising, or point of tenderness Coadministration with other drugs that enhance the effects of serotonergic neurotransmission (eg, tryptophan, fenfluramine, fentanyl, 5-HT agonists, St. John’s Wort) should be undertaken with caution and avoided whenever possible due to the potential for pharmacodynamic interaction (see Contraindications) May cause false-positive urine immunoassay screening tests for benzodiazepines SSRIs and SNRIs are associated with development of SIADH; hyponatremia reported Several SSRIs (eg, fluoxetine, fluvoxamine, paroxetine, sertraline) are metabolized by CYP2D6 CYP2D6 is involved in the metabolism of approximately 20% of drugs in clinical use and displays large individual-to-individual variability in activity due to genetic polymorphisms More than 80 CYP2D6 variant alleles have been identified; however, 4 of the most prevalent alleles, CYP2D6*3, *4, *5, and *6, account for 93-97% of CYP2D6 poor metabolizers CYP2D6*4, the most common variant (~25% frequency in whites), causes a splicing defect; CYP2D6*3 (2.7% frequency) causes a frameshift mutation; and CYP3D6*5 (2.6%) is an entire deletion of the CYP2D6 gene; individuals homozygous for these alleles have no CYP2D6 activity The impact of CYP2D6 activity is further complicated in some SSRIs (eg, fluoxetine, fluvoxamine, paroxetine, sertraline) because in addition to being substrates for CYP2D6, they are also known to moderately inhibit CYP2D6 activity The above information is provided for general informational and educational purposes only. Zoloft prevents the reuptake of the neurotransmitter serotonin, while Cymbalta interferes in the reuptake of the neurotransmitters serotonin and norepinephrine. Sertraline is an antidepressant in a group of drugs called selective serotonin reuptake inhibitors SSRIs. Some young people have thoughts about suicide when first taking an antidepressant. Report any new or worsening symptoms to your doctor. Antidepressant medications are used to treat a variety of conditions, including depression and other mental/mood disorders. These medications can help prevent suicidal thoughts/attempts and provide other important benefits. Tell the doctor right away if you notice worsening depression/other psychiatric conditions, unusual behavior changes (including possible suicidal thoughts/attempts), or other mental/mood changes (including new/worsening anxiety, panic attacks, trouble sleeping, irritability, hostile/angry feelings, impulsive actions, severe restlessness, very rapid speech). My general dr switched my BP medicine to Metoprolol, the generic version of beta blocker Toprol. I started taking it March 1 and since then I've had more asthma epsiodes and symptoms (chest tightness, and lower peak flow meter readings.) I have heard beta blockers especially at higher doses can instigate asthma. Even though I am at Metprolol's lowest dosage, perhaps even this dose is affecting my asthma. (I am not sure when I will get an appointment with my primary dr--either this week or next week so I wanted to ask.) You are definitely in my prayers. When I first went on blood pressure medications, my doctor was very careful about not placing me on a beta blocker since I had asthma. I will be discussing this my pulmonary dr at my next appointment early next week. I had mild, well controlled asthma - 5 years ago I was put on Toprol - starting at 25mgs for HBP. My asthma became awful, with breathing problems every single day. I have to discuss this with my primary and pulmonary drs. At the time, it cost me more in co-pays, but they now have more generics in the other types. I was wondering if anyone has had their asthma act up after using Metoprolol or other beta blockers. Unfortunately for me it took them too long to figure out the Beta Blocker was contributing. I had to be weaned off and it is in my chart that I cannot take beta blockers as they exacerbate my asthma. I am now on low-dose Norvasc, a calcium channel blocker. My pulmonoligist said that even low dose beta blockers can do this to asthmatics or people with airway problems. John319, I would definitely recommend weaning off - just cut them in half and take 12.5 for about 7 days and then you should be able to stop - since its only been a month. I am 30 days off and still struggling with my breathing - I am praying the longer I am off it will get better - however, I am beginning to wonder if being on it so long did permanent damage to my lungs. My advice would be to switch to a different category of HBP meds that does not effect breathing (I heard that Diovan is a good - though I could not tolerate that one) - but many others have had success with that one. I hope that Benicar works well for your HBP control. Good luck to you - and do not wait - good breathing is very important. Thanks for the prayers - you will be in mine too!!!
